What is the Child Fatality Review Program?
Child Fatality Review Program is administered by the Arizona Department of Health Services (ADHS), and utilizes locally developed multi-disciplinary teams throughout the state to conduct a detailed review of the circumstances surrounding childhood deaths.

What are the goals of the
program?
To reduce preventable child fatalities through systematic, multidisciplinary, multi-agency, and multimodality review of child fatalities in Arizona; through interdisciplinary training and community-based prevention education; and through data-driven recommendations for legislation and public policy.

What fatalities do the teams review?
There are 13 local child fatality review teams, with a total of approximately 250 team members, located throughout Arizona. Local teams review the deaths and circumstances surrounding the deaths of all children less than 18 years of age.

How can I obtain information from the reviews?
The State Child Fatality Review Team prepares an annual statistical report on the incidence and causes of child fatalities occurring in Arizona by November 15th of each year. This report is available on the Child Fatality Review website.
Additional statistical information is available upon request to this office, telephone number 602-542-1875. Only aggregate reports are available. Information, which may identify a specific child, will not be released.
